Jax Recovery Homes is a recovery-housing program operated by Shalom House Foundation, Inc., a
nonprofit organization. References to "Jax Recovery Homes," "Shalom House Foundation," "we," "us," or
"our" mean Shalom House Foundation, Inc. and its Jax Recovery Homes program.
This Privacy Policy explains how we collect, use, protect, retain, and disclose information received
through our website, online forms, email, telephone calls, text messages, professional referrals,
donation inquiries, and related communications.
Our residences provide nonclinical recovery housing and peer support. We do not offer detoxification,
medical care, psychotherapy, or licensed substance-use treatment through this website.

We do not use a preliminary website application as a substitute for a complete admission agreement, a clinical evaluation, a medical assessment, or a legally required authorization.
5. Professional Referrals and Applicant Permission
Professionals submitting a referral should provide only the minimum information reasonably necessary
to discuss a potential housing placement. A referring organization is responsible for determining
whether it has appropriate permission or another lawful basis to disclose information to us.
Whenever appropriate, the prospective resident should agree to being contacted before identifying
information is submitted. Submitting a referral does not, by itself, authorize the release of medical
records, counseling records, detailed treatment information, or protected substance-use-disorder
records.
When additional coordination requires an individual's written permission or another specific legal
authorization, we will request appropriate documentation before exchanging information. Referring
professionals should use their own applicable privacy and confidentiality procedures.
6. When We May Share Information
We do not sell or rent applicant, resident, donor, or referral information. We may disclose limited
information when reasonably necessary to:

Authorized employees, administrators, house managers, volunteers, or representatives who need the information to perform their responsibilities and are expected to respect confidentiality.

information, housing status, communications, medication information, screening results, or recovery-related circumstances.
When required or appropriate, we will request the individual's permission before discussing identifiable personal information with a family member or other third party. We may share information without that permission only when a valid legal basis or applicable emergency exception permits the disclosure.
8. Cookies, Analytics, and Website Technologies
Our website and website-service providers may use cookies, log files, pixels, or similar technologies for
necessary site functionality, security, performance, visitor analytics, and other disclosed website
purposes.

Jax Recovery Homes provides nonclinical recovery housing. This website Privacy Policy is not a HIPAA
Notice of Privacy Practices and does not represent that we are a HIPAA-covered entity, a HIPAA business
associate, or a federally regulated substance-use-disorder treatment program.
If a particular relationship, record, referral arrangement, funding source, or program activity is subject to
HIPAA, 42 CFR Part 2, or another confidentiality requirement, the applicable legal requirements and any
required authorization will govern the relevant information. This website policy does not replace any
separate notice, consent, authorization, or agreement that may be required.
17. Emergencies
This website and its forms are not monitored as an emergency response service. If you or another
person is experiencing a medical emergency, overdose, immediate safety threat, or mental-health crisis,
call 911 or contact the appropriate emergency or crisis service.
